Our philosophy of service is to advocate for our senior clients. We work to enhance our client's quality of life, and support and protect family relationships. SeniorCare Family is focused on reducing the stress associated with the transitions of aging, and helping families to maintain a quality of life with aging.

Our Care Management services include a comprehensive geriatric health and home assessment. We create and manage health and service care plans, then implement and coordinate care services. We oversee and monitor care providers and avoid conflict of interest by not directly employing home health aides. We advocate for client goals and wishes and provide educational support for health issues. When facility placement is the best option, we can assist with selecting our client's new home and monitor the client's care in the facility.

We establish a free flow of communication between client, family, physician, and financial care team. Our nurse care managers coordinate and attend medical appointments and most importantly we are on call for our clients 24/7.

The seniorcare family team works to keep our clients safe, independent, and enjoying life, while keeping families informed. Our goal is to provide peace of mind to the people who love the senior in need.

When to Recommend a Geriatric Care Manager:

  • Something "isn't right" with a senior's health and you don't know what to do.
  • You want a professional health care provider to ASSESS the home situation and recommend a care plan.
  • Your family has differing views about how to manage an elderly client's situation and you need a knowledgeable health care professional to act with DIPLOMACY in advocating of the client's best interests.
  • You want to ensure your elderly family member has an ADVOCATE to ensure their rights and wishes are honored.
  • You want to know the elderly client is SAFE in their home or in a facility with on going monitoring.
  • You need GUIDANCE for quality of life decisions for your loved one.
  • You need PEACE OF MIND, knowing a professional is available to respond to the seniors needs 24/7.
  • You need help finding the most appropriate FACILITY PLACEMENT for your loved on.
