Our Certified Case Managers are Licensed Social Workers and Aging Life Care Professionals with years of long term care and case management experience. They are experts in elder care planning and care coordination.
We offer a suite of services that help maximize our clients' independence so they can continue living well in the comfort of their home.

The cost of services is $150 per hour for all work performed, prorated into 10 minute increments.

About Janet Serva
Janet Serva is a NYS Licensed Social Worker and a Certified Case Manager. She is a professional member of the Aging Life Care Association. Janet received her Bachelors of Arts in Gerontology from Molloy College in 1991, and a Master's Degree in Social Work from Adelphi University in 1994. She also completed her Certification in Case Management in 2016. Over the course of her 23-year career Janet's experience includes, but is not limited to, working as the Director of Social Work for several well-respected skilled nursing facilities, conducting home visits for the most vulnerable seniors in the five boroughs and Long Island, assessing clients for mental illness and substance abuse comorbidities affecting their health and functional status, and providing counseling and referrals. She also developed the Social Work Program for one of the first Accountable Care Organization on Long Island, helping to coordinate wrap-around services for Medicare's new health care initiat ives. Janet has been a field instructor for master-level social work interns to help prepare them for the field of geriatric social work, and she continues to provide seniors with care coordination and assistance navigating the ever-changing healthcare system.
