Stonerise provides clinician-led transitional care to help patients get back to health and back to life.

At Stonerise, we define transitional care as warmly and smoothly helping patients transition from one care provider or level of care to another. We offer short- and long-term skilled nursing center stays at all 17 of our transitional care centers, in addition to full-scale home health, therapy and hospice service lines. Wherever we deliver care, we strive to ensure that each patient’s transition is less uncertain, more human and more loving for patients and their families.

Throughout The Stonerise Network, our aim is to increase levels of comfort—physically, mentally and emotionally—and deliver the best health outcomes possible for our patients.
