It is Transition with Care's goal to help seniors live independently at home for a lifetime. We are trained to work with seniors who want to live in their homes for as long as possible. We have the resources to help you create a living environment which will allow you to live comfortably and independently at home by offering the following services for seniors and the families of aging parents:

• We help you find ways to modify your home and coordinate getting the healthcare equipment and in-home services you need to live independently at home for as long as possible.
• We can make sure you or your loved one is receiving the quality care they need, whether your loved one is living independently at home or in an assisted living or nursing home community.
• A healthcare crisis for yourself or an aging loved one can be an emotional and confusing time. We can translate the medical jargon and help guide you through the recovery process.
• We can help you select the right assisted living or nursing home community for your needs, by providing you with the right information to make the right decision.
