Drawing on a solid foundation in care planning for elderly individuals, our team possesses a deep understanding of the complexities involved in navigating the care systems. We strive to bridge the gaps between maintaining independence, in-home care, placement decisions, and offering support to long-distance families or overwhelmed unpaid family caregivers. Our focus is on offering guidance and advocacy to both clients and their families.
Our approach involves assessing care needs, making recommendations for care and assistive devices, and facilitating coordination with various healthcare providers through comprehensive care management. We also provide referrals to other professional services or products that can enhance overall quality of life. Our services encompass conducting home visits, aiding in accessing VA benefits and long-term care insurance, and extending social and emotional support.
In addition, we assist in managing household affairs, which includes reviewing mail, arranging for groceries, coordinating medication refills, scheduling medical appointments, and organizing transportation. Our commitment extends to supporting family caregivers and tailoring personalized tasks in agreement with our clients and their families. We also conduct state assessments to determine appropriate placement in assisted living environments..

Tailored Care LLC is owned and operated by Jen Taylor, a Spokane native with deep roots in the long term care industry in Spokane. Jen Taylor is a graduate of EWU with a Bachelors in Social Work and Aging, a Master's Degree in Geriatric Health Care, and is a nationally Certified Geriatric Care Manager through the International Commission of Health Care. With 25 years of direct care practice in the Spokane community and a vast array of community partners, Tailored Care is the beyond the most experienced and knowledgeable option for you and your loved one.
