Pines to Palms
With a background in Social Work, Pines to Palms Geriatric Care Management helps seniors and aging adults manage their healthcare. Our Care Managers offer a holistic approach by providing an assessment, developing a personalized plan of care, recommend and coordinate resources to create a care team, provide guidance, advocacy, oversight and communication to doctors, other professionals and family members regarding their loved one. Our mission is to provide the greatest quality of life for our clients and help ease the burden of caregivers with a vision to see no gaps in continuity of care for seniors.
Our owner is a Licensed Clinical Social Worker in the states of Michigan and Florida, and a part of the Aging Life Care Association as an Advanced Professional Member, certified in Care Management with over 12 years of experience specific to the field of aging, in non-profit and private settings serving the underserved and uninsured population in addition to private settings.
Our Care Managers have a background in Social Work, Nursing, Physical Therapy, and related fields.
About Malissa Harris, LCSW, CMC
Malissa is a Licensed Clinical Social Worker in the states of Michigan and Florida and holds her certification for Care Management. She as worked with seniors and adults with disabilities in different capacities including non-profit and private sectors, within therapy and care management. Malissa has experience working with individuals battling depression and anxiety by using Cognitive Behavioral Therapy and Solution Focused Therapy. She enjoys witnessing her clients, especially caregivers, find their joy as they are guided to more full-filled lives.
